Rechenformular zur Beschreibung der Vertrauensbereiche des Bayes-Kernels (LR, likelihood ratio).
Ein mathematisches Modell zur schrittweisen Simulation der Einzelprozeduren, die sich bei der Übertragung der kollektiven Testerfahrung
auf die Krankheitswahrscheinlichkeit eines einzelnen Patienten in der klinischen Anwendung eines diagnostischen Tests abspielen
(evidence based diagnostic) .

Eine kurze und angemessene Definition des Begriffes "Bayes-Kernel" als Kerngrösse des Bayes-Theorems zur mathematischen Beschreibung der Diskriminationskraft eines Tests könnte etwa folgendermassen lauten: Der Bayeskernel, genannt "likelihood ratio (LR)", ist ein doppelseitiger, zweidimensional vernetzter RR-OR-Operator, der in der vertikalen Ebene als risk ratio (RR) der Disease attributablen Risiken einen positiven Test (pLR) oder negativen Test (nLR) aufzuweisen definiert wird und gleichzeitig in seiner Kernfunktion als hazard ratio (HR) der posttest/pretest Krankheitsprobabilität agiert als odds-Operator zur Berechnung der Test attributablen Krankheits-Oddsprobabilitäten in der horizontalen Ebene. Diese komplizierte und schwer verständliche Sentenz lässt sich mathematisch formulieren mit den simplen Gleichungen pLR = SE / [1−SP] ≡ PPVodds / PVodds ; nLR = [1−SE] / SP ≡ [1-NPV]odds / PVodds .
In dieser Tabellenkalkulation wird am statistisch-mathematischen Modell die Übertragung der
Qualitätsdaten eines Testreferenz-Kollektivs (Sensitivität, Spezifiät) auf das prätest-Krankheitsrisiko einer Einzelperson simuliert − ein in der klinischen Diagnostik und Prognostik alltäglicher Vorgang beim Abschätzen der individuellen posttest-Krankheitswahrscheinlichkeit eines einzelnen Patienten anhand eines Testresultats.
Das Bayes-Theorem zur Beschreibung Testreferenz-Qualität wird in der Null-Hypothese H0 (Definitionsebene, Tabelle 1) formuliert. Die Berechnung der posttest-POD (Krankheitswahrscheinlichkeit) mittels der likelihood ratio (LR = "Bayes-Kernel") in der Fagan-Kalkulation erfolgt in der Arbeitshypothese HA (Funktionsebene des Bayes-Kernels, Tabelle 2). Die strikte Trennung von Definition und Funktion der likelihood ratio ermöglicht es den komplexen Vorgang der Bayes-Fagan-Kalkulation eines diagnostischen Tests in Einzelschritten mathematisch nachzuvollziehen .
Dieses mathematische Modell liefert die rationale Begründung, warum für das Konfidenzintervall des zweidimensionalen, hypothesenübergreifenden Bayes-Kernels in seiner Fagan-Funktion die Konfidenzvarianz des relativen Risikos (RR-Varianz) der Defintionsebene und nicht die Konfidenzvarianz der hazard ratio (HR-Varianz, HR ist die odds ratio OR horizontaler odds) der Funktionsebene verwendet werden sollte .

A short and appropriate definition of the Bayes kernel would be : The Bayes kernel, called likelihood ratio (LR) is a doubelsided, two dimensional networking RR-OR-operator, defined in the vertical direction as the risk ratio (RR) of the disease attributed risks to have a positive test (pLR) or to have a negative test (nLR), but acting simultaneously as the odds ratio (OR) posttest/pretest probability of disease to compute the test attributed odds-POD in horizontal direction. . This complicate and hardly to understand sentence is mathematically formulated by the simple equationspLR = SE / [1−SP] ≡ PPVodds / PVodds ; nLR = [1−SE] / SP ≡ [1-NPV]odds / PVodds .
This statistic-mathematical model - a tablecalculation program - simulates the transmission of the quality parameters (sensitivity, specificity) from a testreference cohort to the pretest probability of disease (pretest-POD) of a single person - a most common procedure in clinical use of a diagnostic test to estimate the individual posttest-POD of a patient .
The Bayes-Theorem, used to describe the quality of the testreference, is mathematically formulated in the zero-hypothesis H0 (table 1, dimension of definition, definition of the likelihood ratio LR). The functional dimension of the LR, used in the Fagan-calculation to estimate the posttest-POD depending on the pretest-POD, is formulated in an alternative working hypothesis (HA, table 2, Bayes-kernel in function). The strict separation of the Bayes-kernel definition and function allows it, to describe the complex procedure of the Bayes-Fagan-calculation step by step in a simple mathematical model .
This model illustrates the rational reason, why the confidence interval of the two dimensional Bayes kernel (= LR, likelihood ratio) functioning as the operator of the Fagan calculation should be based on the variance of the risk ratio (RR-variance) of the disease attributed risks (pLR = SE/[1-SP] and nLR = [1-SE]/SP) described by the Bayes theorem (see table 1, dimension of definition) and should not be based on Woolf's variance for the hazard ratio (HR-variance) posttest-/pretest-POD in the functional dimension of the Fagan calculation (table 2, working hypothesis HA). For the individual posttest-POD of a patient the confidence interval based on the Woolf's HR-variance is so enormously large, that one would not trust the Fagan calculation any more.
